ADMISSION FOR
CLASS-1 & II (For the session 2012-13)
A.
Forms will be
distributed from 15th February 2012 to 20th March 2012 free of cost.
1.
Timing
:9:00AM to 11:00 AM (only on working days)
2.
Attested
xerox copies of Birth Certificate issued by Competent Authority
should be shown for collection of form.
3.
Those who
want brochure /prospectus have to pay
` 100/- along
with an application.
B.
Date of
registration for classes I & II
1.
Filled in
registration form will be accepted from 15-02-2012 to 20-03-2012
(Timing 1145 Hrs to 1300 hrs only on working days).
2.
Last date i.e
on 20-03-2012, registration forms will be accepted up to 1700 hrs.
C.
Papers to be
attached with filled in form:
1.
Service
certificate (Page 2 of the form).
2.
Transfer
details (Page 2 of the form) with distance and period of stay.
3.
Xerox Copy of
Birth Certificate (attested).
4.
SC/ST
candidate (certificate of either parent should be attached).
5.
Certificate of ex-servicemen
(Defence) from soldier & sailor board.
6.
Residential
proof certificate (electricity bill, certificate of Counsellor or
Gram panchayat, Tax receipt of KMC, Agreement with Landlord for
rented accommodation.
7.
Self
declaration of distance from residence to KV Joka ,Kolkata-104
8.
Physically
handicapped candidate should attach certificate from Civil Surgeon/
any other authority or rehabilitation centre.
9.
BPL (Below
Poverty Line) candidate to attach the Xerox copies of ration card .
10.
Income
certificate from Department/Revenue officer.
11.
Certificate
of Creamy layer in case of SC/ST/OBC.
12.
Certificate
of Single Girl Child.
D.
Declaration
of list and Admission for Class-I-
26-03-2012 to 31-03-2012
E.
Declaration
of list and Admission for Class-II-
16-04-2012 to 27-04-2012
Incomplete forms without
supporting documents will be rejected .
